ABSTRACT

OBJECTIVE:To study general chara cteristics and medication of medical damage liability disputes cases caused by medication error , and to provide references for related departments and medical staff for preventing and reducing medication-induced medical disputes. METHODS :A total of 240 cases of medical damage liability disputes cases caused by medication error were collected from Peking University ’s Fabao Law Database during Jan. 2001 to Feb. 2020,and analyzed in terms of general situation ,damage outcome ,level of the hospital involved ,liability judgment and compensation ,types of medication error and drug types. RESULTS :medication-related medical damage liability disputes accounted for 25.3% of overall medical damage disputes ;the most damage result of patients was death (68.3%);medical negligence forensic appraisal was conducted as the main appraisal pattern with a proportion of 57.9%;the average case compensation was 203,000 yuan;the hospitals involved were mainly tertiary hospitals (48.8%);the main type of medication error involved was prescription error ; chemical medicine was mainly involved ,of which the top three categories were systemic antibacterial ,systemic corticosteroids and antipsychotics. CONCLUSIONS :ADR caused by medication errors are the common causes of medical disputes. Medical institutions should focus on improving the relevant systems and processes ,strengthen the construction of pharmaceutical information and automation system ,and reduce the probability of medication errors ;at the same time ,great importance should be paid to the cultivation of pharmaceutical talents in hospital ,give full play to the role of pharmacists ,and strengthen the monitoring and intervention of medication errors. Finally ,the relevant national judicial departments should constantly improve the settlement mechanism of medical damage liability disputes to provide reasonable protection for both doctors and patients.

ABSTRACT

OBJECTIVE:To e xplore the health economic burden of hypertension patients at county-level areas and its influential factors in China. METHODS :A questionnaire survey was conducted on hypertension patients in 7 county-level public hospitals from 6 provinces as Hebei ,Shandong,Shanxi provinces by using a convenient sampling method. Catastrophic health expenditure was defined by the standard of “medical and health expenditure exceeding 10% of household income ”. The incidence , average gap and relative gap of catastrophic health expenditure were analyzed. A multi-factor Logistic regression model analysis was used to analyze the influential factors that lead to catastrophi c health expenditure. RESULTS :A total of 1 378 questionnaires were sent out ,and 925 valid questionnaires were collected with effective rate of 67.13% . The incidence of catastrophic health expenditure,average gap and relative gap among hypertension patients were 23.03%,19.37% and 84.12%,respectively. At different income levels ,the incidence of catastrophic health expenditure,average gap and relative gap were 72.67% , 96.79% and 133.18% in the poorest household group ,and were 1.94% ,0.47% and 24.23% in the richest household group. Among different types of medical insurance ,the incidence of catastrophic health expenditure in patients covered by “New Rural Cooperative Medical Scheme (NRCMS)”the highest (31.30%). The household income ,complications and the type of health insurance had significant impacts on the incidence of catastrophic health expenditure in hypertension patients (P<0.05). CONCLUSIONS:The incidence of catastrophic health expenditure in hypertension patients with different income levels is different. As the income level raised ,the incidence of catastrophic health expenditures continued to decrease. But the protection of household health expenditure by NRCMS is weak. It is suggested that a certain policy preference should be given to families with low income and patients with chronic diseases ,so as to ensure the rights and interests of patients with hypertension .

ABSTRACT

OBJECTIVE: To provide experience and reference for the study of medical insurance budget impact analysis (BIA) in China. METHODS: Retrieved from PubMed, ProQuest, CNKI, Wanfang database and CBM, related literatures about medical insurance BIA research in China and the United States were collected since the establishment of the database. The basic information, analysis results and data sources were summarized and sorted out, and descriptive analysis of the included literature was carried out on basis of seven key elements such as model design, research perspective, treatment cost, reference scenario, target population, research time limit and discount/inflation, sensitivity analysis. RESULTS: A total of 72 literatures were included in this study, involving 24 (33.33%) studies in China, 48 (66.67%) studies in the United States; the indications of 45 studies were chronic diseases (62.50%), and those of 21 studies were acute diseases (37.50%). Among the research methods, 49 studies (68.06%) used BIA alone and 23 studies (31.94%) adopted BIA combined with pharmaceutical economics. In terms of model design, 50 studies (69.44%) adopted cost calculation models. In terms of research perspective, 60 studies (81.94%) were based on the perspective of medical insurance department research. In the calculation of treatment cost, 69 studies (95.84%) included drug cost. In terms of reference scenarios, 61 studies (84.72%) compared the economics of different drug-based treatment groups. For target population, only 31  (43.06%) studies used real world data. In terms of research duration and discount/inflation, 14 studies (19.44%) used treatment or length of hospitalization to indicate research duration, and 19 studies (26.39%) used discount rate or inflation rate to adjust costs. As for sensitivity analysis, 62 studies (86.11%) conducted sensitivity analysis, of which 49 (68.06%) used single factor sensitivity analysis. CONCLUSIONS: There are still some limitations in medical insurance BIA research literature in China and the United States, such as unreasonable use of data, incomplete coverage of the cost, and unreasonable setting of sensitivity analysis variables. It is recommended that BIA research should standardize data sources to improve the quality of budget evidence quality, reasonably evaluate market size to improve the authenticity of prediction, scientifically set variables and their scope of change to improve the stability of results, establish BIA research paradigms or evaluating standards so as to guide BIA research scientifically.

ABSTRACT

OBJECTIVE:To put forward relevant suggestions for formulating and implementing medical insurance budget impact analysis(BIA)research guideline in China. METHODS:The medical insurance BIA guidelines or relevant documents were retrieved from ISPOR,Canada and the United States. Their similarities and differences were compared. The experiences of them were summarized in respects of research framework,data source and report format of medical insurance BIA. RESULTS &CONCLUSIONS:ISPOR,Canada,USA and other guidelines or relevant documents have some similarities in normalization requirements of research perspectives,target population calculation based on natural reason and epidemiological data,3-5 years as the research time limit and some other respects. But the calculation of additional costs as drug price addition,distribution fee should be specifically adjusted according to the unique characteristics of the health systems of countries or regions. Based on the actual conditions of our own health care system,our country can draw up the guidelines for medical insurance BIA impact analysis,which contain model design,research perspective,target population,current use of intervention measures,prediction on the effects of new intervention measure introduction on the market,cost,time range,discount and uncertainty analysis of current and new interventions,situational analysis and verification,so as to better play its role in the formulation and adjustment of medical insurance catalogues and in drug price negotiations.

ABSTRACT

OBJECTIVE:To evaluate the feasibility and effectiveness of Hazard analysis and critical control point (HACCP) system in the process of production of Loratadine tablets. METHODS:Taking wet granulation and tableting technology of Lorata-dine tablets as an example,and through the introduction of the concept of HACCP,the basic theory and method of HACCP were applied for hazard analysis on each production link to find critical control points and set critical limits for production quality man-agement. RESULTS:By HACCP analysis,three links namely drying,granules fitting and mixing,internal and external packaging were finally determined as the critical control points in the process of production of Loratadine tablets,thereby critical control lim-its were set for monitoring. After effective control over the risks in the process was ensured,HACCP work plan was made and veri-fied,and the results showed that HACCP system could effectively control and reduce the risks in the process of production and en-sure quality safety. CONCLUSIONS:Application of HACCP system to wet granulation and tableting technology of Loratadine tab-lets can fully embody its feasibility and effectiveness.

ABSTRACT

OBJECTIVE To explore the suitable hospital infection control measures in health centers of poverty-striken villages,in order to improve the management of hospital infection,decrease hospital infection rate and protect the health of medical staff and patients.METHODS The status quo of hospital infection in health centers of poverty-striken villages,was investigated in 20 small towns health centers with were randomly divided into two groups:test group(n=15)and control group(n=5).The suitable hospital infection control measures were explored from 5 points.The effect of infection control by before-after controlled study of experimental group and randomized controlled study of control group was anal yzed.RESULTS The rate of hospital infection in test group was decreased from 7.60% to 1.98% and at in control group didn't change,the difference was significant.CONCLUSIONS The managements of establishment of the suitable hospital infection control measures in health centers of poverty-striken villages have been put into practice and gained good result.
